Welcome to Relay Utah

The Relay Utah program provides specialized telephone equipment such as amplified volume control telephones, captioning telephones, neckloops, in-line amplifiers, and mobile phone options. Equipment is provided at no cost for qualified individuals who are Deaf, Hard of Hearing, or Speech-Challenged.

Eligibility requirements

The applicant must:

  • reside in the state of Utah;
  • be deaf, hard of hearing, or speech-challenged; and
  • receive assistance from a public assistance program administered by a state agency, or have an income of 200% or less of the federal poverty guideline.

Relay Utah Privacy Notice

This Privacy Notice describes how Relay Utah, a program administered by the Public Service Commission of Utah (PSC), handles personal information that you provide to us, what information is disclosed, with whom the information is shared with, and how you can request access to this information under the Government Data Privacy Act (GDPA), Utah Code Title 63A, Chapter 19. In addition, more restrictive state and federal laws may apply.

What Personal Data do you collect?

Relay Utah collects limited personal data from individuals who receive telecommunications equipment used for individuals with hearing loss and speech challenges. This information may include:

  • Names: Full name of the applicant, and if applicable, parent, guardian, or authorized representative.
  • Contact Information: Home or mailing address, phone numbers, and email addresses.
  • Documentation: To qualify for specialized telecommunication equipment for individuals who are deaf, hard of hearing, or speech impaired, documentation collected may include the following:

1. Evidence of receiving assistance from a low-income public assistance program administered by a Utah state agency; or has an income of 200% of the federal poverty guidelines for the current year under R746-8-405(3), Subsection R746-8-405(3)(iii)

2. Certification that the individual is deaf, hard of hearing, or speech impaired under R746-8-405(3), Subsection R746-8-405(3)(a)(ii).

We may collect this information through forms, email communications, and online services. We may collect your name, email address, phone number or other personally identifiable information to satisfy the intended purpose of your submission. Any sensitive information, including account numbers and Social Security numbers, should be redacted from your documents before submitting them to Relay Utah.


Relay Utah contracts with T-Mobile Accessibility for Traditional Relay Services (TRS). TRS allow persons who are deaf, hard of hearing, deafblind, or have speech disabilities to communicate by telephone in a manner that is functionally equivalent to telephone services used by persons without such disabilities. Relay Utah does not have access to and does not collect or store the content of any TRS conversations, transcripts or applications. All TRS communications are confidential under federal and state law.

How do you use my Personal Data?

Relay Utah uses your personal information to:

  • Verify eligibility for specialized telecommunication equipment.
  • Deliver and maintain specialized telecommunications equipment.
  • Provide customer support and technical assistance.
  • Maintain records for state and federal reporting and compliance when required.
  • Improve accessibility and service quality.

Relay Utah complies with the Government Records Access and Management Act (GRAMA) and other applicable federal and state privacy protections. Personal information collected is considered private, controlled, or protected data.


What happens if I do not want to provide my personal data to Relay Utah?

If you choose not to provide the information requested, Relay Utah may not be able to process your application for specialized telecommunication equipment.

We do not sell or lease your personal information to third parties. Your information is only shared with authorized personnel or third-party service providers who require it to perform specific tasks related to functionality or services. We may use cookies that are necessary for site functionality to provide you with the best experience. We implement reasonable security measures to protect your information from unauthorized access or disclosure.
